3. Cast Certificate / Cast validity certificate / Non Crème Layer for Backward Class (if required) 
4.  Gazette Notification (if name on documents are different than the name in which you want     to apply)
5. Recent passport size coloured photograph (35 mm X 45mm) .72 to 150 DPI resolution.     ( File    format .jpg format,)
